Our locations

Delivering our services within the home environment or locally within the community helps us to deliver more person-centred care, which in turn boosts independence, protects wellbeing, and safely avoids unnecessary hospital admissions.听 This is especially important for our most rural communities and those who may find it difficult to access our services.

We are proud to offer a wide range of community services across East, North and Mid Devon including Exeter, delivering nursing, rehabilitation and wound care services and much more. These are delivered at a locality level based around GP practices within our Community Health and Social Care clusters.

We work in partnership with Devon County Council and other organisations to ensure our patients, carers and families experience the best possible integrated health and social care service. What services do we provide?

We provide the following services within the community:

  • Community Nursing service for adults and children
  • Integrated health and social care
  • Rehabilitation and recovery support
  • Occupational therapy and physiotherapy
  • Diabetes foot care and community podiatry
  • Skin and wound care including leg ulcer management
  • Sexual Health services
  • Mobility and equipment services
  • We also provide a range of acute medical, specialist and surgical services within our community hospitals

North Devon (Northern Services)

Our service is split across North Devon and stretches all the way to Bude and Holsworthy, serving a wide population across our community settings. We often refer to individual areas as 'clusters', which helps us provide the right care, at the right time, in the right setting.

We currently have the following clusters:

  • Ilfracombe and听 Braunton听
  • South Molton and Chumliegh
  • Barnstaple听
  • Torridge (Bideford, Northam, Torrington, and Hartland)
  • Holsworthy, Bude and Surrounding Villages

East and Mid Devon and Exeter (Eastern services)

Our service is split across East and Mid Devon and Exeter, serving a wide population across our community settings. We often refer to individual areas as 'clusters', which helps us provide the right care, at the right time, in the right setting.

We currently have the following clusters:

  • Crediton, Moretonhampstead and Okehampton
  • Honiton, Ottery St Mary and Cranbrook
  • Exmouth, Woodbury, Budleigh
  • Exeter 鈥 South and West
  • Exeter 鈥 Central and East
  • Tiverton and Cullompton
  • Sidmouth, Seaton and Axminster

How can I be referred?

Most referrals are made by a relevant Healthcare Professional such as your GP, or via our Acute services if you receive care at hospital or another setting. We also work in partnership with organisations including Devon Country Council and other health and social care providers, who may help you access our services.听

You may be able to self-refer to some services, but please contact your GP to find out more.听
